Required Application Steps

Step 1: Application Form

After you have completed the application and parent perspective form, please return them to City Academy along with your child’s birth certificate and social security card.

Step 2: Release of Information, K-6th

Please send the Release of Information Forms to your child’s current school. Request that a copy of all transcripts and test scores, from kindergarten to present grade, be sent to City Academy. Applications will not be considered complete without this information. This information must be sent to City Academy directly from the school and will not be accepted if submitted by applicant’s family.

Step 3: Assessment and Parent Interview

As part of the application process, your child will complete an assessment at City Academy. This process will allow us to assess your child’s social and academic readiness for our program. Assessments and parent interviews are scheduled on weekday mornings for Early Childhood students. Grades second-sixth must do testing on February 15.

Step 4: Financial Aid Form

The applicant’s parents must submit their most recent Form W2 (Wage & Tax Statement) and Form 1040 (U.S. Individual Income Tax Return) must be submitted with the form.

Step 5: Notification of Decision

All decision letters will be mailed on March 15, 2019.
